Generic Service Summary Supplement Definition


The Generic Service Summary Supplement validates strong processes, procedures, and structures for programs that are not being reviewed under a specific COA service section.


This supplement should be completed for any program that is not being reviewed under a specific COA service section. Speak with your COA Coordinator prior to completing this supplement to determine its applicability to your program.

Note: The following documents must be available on-site for review:

  • Outreach strategies and informational materials
  • Job descriptions
  • Training curricula
  • Documentation of training attendance

Additionally, the following activities will be conducted during the site visit:

  • Interview:
    1. program director
    2. relevant staff
    3. individuals and families served
  • Review unofficial employee files
Self-Study Narrative Evidence
  • Provide an overview of the different services being accredited under this section. The overview should describe:
    1. the service philosophy and approach to delivering services;
    2. eligibility criteria; and
    3. any unique or special services or supports provided to specific populations.
  • Address staffing patterns and staff competencies by providing:
    1. Service-specific staffing chart that includes lines of supervision.
    2. List of program personnel that includes:
    3. name;
    4. title;
    5. degree held and/or other credentials;
    6. staff classification (e.g.contractor, government employee, FT/PT, volunteer, etc.);
    7. length of service at the program; and
    8. time in current position.
  • Table of contents of service-specific training curricula not already addressed in MIL-HR 2
  • Provide any other information you would like the Peer Review Team to know about this program.
  • As applicable, a list of activities or classes including, for each activity or class:
    1. the type of activity/class;
    2. whether the activity/class is short-term or ongoing;
    3. how often the activity/class is offered;
    4. the average number of participants per session of the activity/class, in the last month; and
    5. the total number of participants in the activity/class, in the last month
  • A list or description of service-specific outcomes and outputs being measured
