The Generic Service Summary Supplement validates strong processes, procedures, and structures for programs that are not being reviewed under a specific COA service section.


This supplement should be completed when the organization has opted to include a program in its accreditation review that does not fit under a specific COA service section. When multiple programs are being reviewed using this supplement, each piece of submitted evidence must be combined into one document for upload or one narrative. Speak with your COA Coordinator prior to completing this supplement to determine its applicability to your organization.

Note: The following documents must be available on-site for review:

  • Outreach strategies and informational materials
  • Job descriptions
  • Training curricula
  • Documentation of training attendance

Additionally, the following activities will be conducted during the site visit:

  • Interview:
    1. program director
    2. program personnel
    3. individuals and families served
  • Review personnel files
  • Review case records, if applicable

Generic Service Summary Supplement Narrative

Self-Study Evidence
    • An overview of the program that describes:
      1. the program’s name, service philosophy, and approach to delivering services;
      2. eligibility criteria;
      3. any unique programs or special services provided to specific populations; and
      4. major funding streams
    • If elements of the service (e.g., assessments) are provided through contract with outside programs or through participation in a formal, coordinated service delivery system, provide a list that identifies the providers and the service components for which they are responsible. Do not include services provided by referral.
    • Address staffing patterns and staff competencies by providing:
      1. Program staffing chart that includes lines of supervision
      2. List of program personnel that includes:
        1. name;
        2. title;
        3. degree held and/or other credentials;
        4. FTE or volunteer;
        5. length of service at the organization;
        6. time in current position
        7. Table of contents of training curricula
    • Provide any other information you would like the peer review team to know about the program.
    • A list of all program sites, including the following information for each program site:
      1. name of program/site director;
      2. address;
      3. phone number;
      4. hours of operation;
      5. number of FTEs;
      6. average monthly number of clients served; and
      7. directions or a map to each program site from the main organization office
    • A demographic profile of persons and families served by the program being reviewed with percentages representing the following:
      1. racial and ethnic characteristics;
      2. gender/gender identity;
      3. age;
      4. major religious groups; and
      5. major language groups
    • As applicable, a list of groups or classes including, for each group or class:
      1. the type of activity/group;
      2. whether the activity/group is short-term or ongoing;
      3. how often the activity/group is offered;
      4. the average number of participants per session of the activity/group, in the last month; and
      5. the total number of participants in the activity/group, in the last month
    • A list or description of program outcomes and outputs being measured
